How to File a Complaint

If an employee or beneficiary of a subrecipient (individual receiving services supported by a

federal or state grant) believes that a subrecipient (organization supported by a federal or state

grant) has discriminated against him or her, that individual may file a complaint of

discrimination with the subrecipient (Texas Forensic Nurse Examiners (TXFNE), the OOG, or the

OCR/CRCL as appropriate.

Complaints must generally be filed within 180 days of the alleged discrimination, or within one

year of the alleged crime for allegations under the Omnibus Crime Control and Safe Streets Act,

the Victims of Crime Act, or the Violence Against Women Act. You can read more about these

programs online.

Concerns should first be voiced at the time, if possible. You may ask to talk to a Supervisor at

any time. It is our goal to treat each patient with dignity and respect and we are dedicated to

providing excellent, patient- centered care.

You may make a complaint to our organization in writing to either khara@txfne.com or

julie@txfne.com If you would like to escalate your concern to the Office of the Attorney

General you can download a complaint form at https://gov.texas.gov/organization/cjd and

submit the form to GAD@gov.texas.gov. You may also contact the Office of /Civil Rights and

Civil Liberties at (800) 421-3481 or email them at OCR@ed.gov. Directions for filing a complaint
